Athletic Clearance & Sports Physical Information for the 2025–2026 School Year

If your student plans to participate in sports, band, dance, or cheer during the 2025–2026 school year, please review the important information below regarding required Sports Physicals and the Online Athletic Clearance process.

🏥 SPORTS PHYSICALS

All student-athletes must have a valid Sports Physical completed after June 1, 2025 to be eligible for participation.
  • Physicals must be performed by a licensed MD, DO, PA-C, or NP.
  • No athlete may try out or practice without a current physical on file.

💻 ONLINE ATHLETIC CLEARANCE

The Online Clearance Packet must be completed and submitted before any athletic participation (including tryouts).
  • The process is completely electronic—no hard copies will be accepted.
  • You must upload your signed physical form directly to your Home Campus account.
